 ENTRY WAIVER OF LIABILITY and PHOTOGRAPHY:  I, the undersigned, as group representative, hereby
release, remise and forever discharge Unity Community Development Corporation, its officers and directors,
severally or jointly, the Unity Black Pride Parade Committee and all volunteers of and from any and all liability,
claims, actions and possible causes of action which may accrue to any member of the group from every and
any loss, damage and injury (including death) that may be sustained while participating in the picnic or parade.
And WAIVER OF PHOTOGRAPHY/VIDEO RELEASE: I authorize the Unity Black Pride Parade and Picnic
Committee to use photos, and or other likenesses of myself or organization for any promotional materials
regarding Black Pride Parade and Picnic programs. Such likenesses will not be sold to other parties.
Promotional materials bearing these likenesses may be distributed for free to the public and posted on the
Unity CDC website. By inserting "yes" in the waiver reply, you agree with the waiver.
